Healthy Eating JUMPSTART

The Healthy Eating JUMPSTART program provided a Shipt.com grocery shopping credit of $80 per month, allowing eligible low-income individuals to shop online for lower-carb groceries and receive their Healthy Eating JUMPSTART delivered to their home for free.

Along with groceries, patients were given access to robust education and support for lower carb eating, including handouts, tips, recipes, meal plans, and more.

HBOM Tobacco Cessation Provider Kit

Making every contact count (MECC) is an approach to behavior change that is clinically proven to help patients quit smoking and have better health outcomes. The Tobacco Cessation Provider Kit includes a set of materials designed to make following MECC and offering tobacco cessation support easy.

Clinic Poster Design Challenge: Tobacco Cessation and Type 2 Diabetes

Providers from around the state of Michigan submitted their ideas for posters they wanted to see in their clinics on two health topics: tobacco cessation and type 2 diabetes. Winning ideas were selected and transformed into beautiful, eye-catching posters.

The Michigan Collaborative for Type 2 Diabetes data dashboard displays health information for all patients in Michigan who have been diagnosed with type 2 diabetes.
