MCT2D Learning Community for Primary Care Providers
Overview
The MCT2D Learning Community acts as an engine for driving engagement and collaboration within the collaborative. It provides opportunities for members to connect with each other, share feedback and ideas, and participate in educational opportunities all focused on type 2 diabetes care.
As a member of the design and informatics team, I helped conceptualize and pitch the design for the Learning Community. I also help create and host many of the activities that are foundational to the community.
Community Model
Regional Identities
As part of the Learning Community, each of MCT2D's 7 regions across the state of Michigan were asked to help create a regional name and identity. Over 300+ clinicians voted on a mascot and name for their region, which I then used to create regional seals.
Member Dashboard
MCT2D is building a member dashboard to help members find Learning Community events, participation opportunities and track their completion of community requirements. I created the design for the layout and functionality of the members' dashboard. The final version will release in late 2023.
